The Power of Pelvic Categories: Why Your Pain Might Start at the Foundation

Most people don’t realize how much the pelvis affects their entire body—until something goes wrong. Whether it’s recurring low back pain, nagging hip discomfort, instability when walking, or even digestive and bladder issues, the pelvis is often at the root. In Applied Kinesiology, we recognize distinct Pelvic Categories—specific patterns of dysfunction in the sacroiliac joints and surrounding tissues. These aren’t just arbitrary classifications—they’re reliable, testable, and treatable frameworks for understanding why the body is compensating, and more importantly, how to correct it. Let’s take a look at these three powerful patterns and what they can reveal.
Category I: The Subtle Twist
What it is: Category I represents a torsional distortion—usually a symmetrical imbalance in how the sacrum moves with the ilia. It’s subtle but profound, often going undetected in standard exams.
Common symptoms:
Low-grade back tension or fatigue
Brain fog or trouble focusing
Chronic headaches or jaw tension
Shifts in cranial rhythm or TMJ
A sensation of being “off” but hard to pinpoint
Why it matters:
This isn’t just about the pelvis—it’s about the craniosacral system. The dura mater, which surrounds the spinal cord and brain, connects directly to the sacrum. If there’s a twist in the pelvis, it creates tension all the way up the spine and into the head.
How we correct it:
Gentle pelvic blocking, cranial release, and breath-guided muscle work help unwind the system. We don’t force the fix—we let the body recalibrate.
Category II: The Unstable Pelvis
What it is: Here we see one side of the pelvis shifted forward or backward, creating instability in the sacroiliac (SI) joint. One leg might feel longer, movements feel uneven, and there’s often pain with standing, walking, or changing positions.
Common symptoms:
Sharp low back or hip pain
Clicking in the SI joint when walking
Groin pain or radiating discomfort into the leg
Recurrent “throwing out” of the back
Pelvic floor tension or weakness
Why it matters: This is a mechanical and neuromuscular problem. The ligaments meant to stabilize the pelvis are under strain, and the muscles compensating for it are working overtime. It’s a pattern we often see in athletes, new moms, or anyone with repetitive movement or trauma.
How we correct it:
We use specific muscle testing to find the imbalances, stabilize the ligaments, and often employ SI blocking techniques to gently reset the joint. Activating the core and glutes is essential for long-term stabilization.
Category III: The Structural Breakdown
What it is: Now we’re dealing with disc involvement, most often at L5/S1. This is where the body’s warning system is in full effect—pain, inflammation, nerve tension. The sacroiliac joint may have already gone through the instability of Category II and collapsed into this more severe state.
Common symptoms:
Sciatica or nerve pain down the leg
Difficulty bending forward or standing upright
Disc herniation or degeneration
Intense low back pain with muscle guarding
Loss of mobility or strength in one leg

Category III is serious—but not hopeless. We gently decompress the spine using pelvic blocks and manual alignment techniques, then retrain the stabilizing muscles. Neurological re-patterning and fascia release help the body find safety and strength again.
